Job Safety Analysis Template

Purpose. Supervisors and crews use this template to break a non-routine task into steps, identify the hazards of each step and agree the controls before work begins.
1.When a job safety analysis is required
A job safety analysis must be completed for any task that is not covered by an existing procedure, any task involving working aloft, over the side, in a confined space, with hot work, or with lifting gear, and any task the supervisor considers unusual.
2.Breaking down the task
List the steps in the order they will be done. Keep each step to a single action so that hazards are not missed.
3.Identifying hazards
For each step, identify what could cause harm: falls, crushing, snap-back, fire, fumes, drowning, weather, plant movement and fatigue. Consult the crew who will do the work.
4.Controls
Apply the hierarchy of controls: eliminate the hazard, substitute, isolate, engineer, then administrative controls and personal protective equipment. Name the person responsible for each control.
5.Sign-on and review
Every crew member involved must sign on to the analysis after the briefing. The analysis must be reviewed if the task, the weather or the crew changes.
