Purpose. Crew use this form to report a defect in machinery, hull, equipment or safety gear so it can be entered in the planned maintenance system and assessed by engineering.

1.When to use this form

Complete this form as soon as a defect is found, whether or not the vessel can continue operating. Give it to the master, who must enter it in the planned maintenance system the same day.

2.Describing the defect

State the equipment, its location, what is wrong, when it was found and what the readings or symptoms are. Attach photographs where they help. Do not attempt a repair that is beyond your competence.

3.Safety impact

The master must decide whether the vessel can sail with the defect and record the decision. Defects affecting steering, propulsion, watertight integrity, fire equipment or lifesaving appliances must be treated as critical until engineering assesses them.

4.Temporary measures

Record any temporary repair or operating restriction applied, such as reduced speed or a tank isolated, so that relieving crews are aware.

5.Engineering assessment

The Fleet Engineering Manager will classify the defect and schedule the work. The reporting crew member will be told the outcome.
